APPLYING FOR MEDQUEST HEALTH CAREER EXPLORATION CAMP Applying for MedQUEST Health Career Exploration Camp is easy! If you are eligible (currently in Grade 10 or Grade 11) and decide you would like to participate in MedQUEST Program: - Discuss your intentions with your high school guidance counsellor, principal, vice-principal, science teacher or another who knows you well. Student applications must include a student referral form from your high school. An application submitted without this information will not be assessed.
- Discuss your plans with your parents/guardians. Transportation to and from camp, vacation/work schedules, camp fees, job shadow opportunities and proof of immunization details will need to be discussed, and determined reasonable for you and your family.
- Be mindful of our deadline of April 20th, 2010. Due to the volume of applications, there will be NO EXCEPTIONS.
1. WHAT IS THE FEE FOR MEDQUEST? The fee for the program is $200 per student and includes meals*, excursions and activities for all days of camp. Students also receive a Schulich School of Medicine and Dentistry scrub top for their job shadow placements, as well as a welcome kit and printed materials. Financial assistance may be available based on need. (*Meals may not be provided on job shadow placement days. It may be more feasible to bring a prepared lunch.) 2. HOW DO I APPLY FOR MEDQUEST? Students must submit a completed on-line application, which includes the following: - Student Application Component (your personal information).
Student Reference Component (your recommendation from your guidance counsellor, principal or vice-principal, classroom teacher, or coach). Student and Parental Consent Component (your understanding of the requirements). Student Code of Conduct Component (your understanding of expectations) - This component will be completed by yourself and your parent/guardian
- Student Health & Immunization Component (your up-to-date health information, including 2-step TB skin test)
- This component will be completed by yourself and your parent/guardian.

3. WHEN IS THE APPLICATION DEADLINE? The APPLICATION DEADLINE is April 30th, 2010. There will be NO EXCEPTIONS. 4. HOW ARE STUDENTS SELECTED? A selection committee will meet to review all applications shortly after the deadline. It is vital that all information be received prior to the deadline. Acceptance notification will be sent out by May 7th, 2010. Students will receive a confirmation via their email address. Further, notification will be forwarded to your parent/guardian, as well as your student reference.
The full camp fee is due within ten days of receipt of the acceptance letter and info package. Payment may be made online via Mastercard or Visa. If alternate arrangements are required, please contact the MedQUEST Coordinator.
Please note: A valid email address is required as most information is forwarded electronically, from the Central Office and also from MedQUEST medical student counsellors.
7. ARE THERE ANY SCHOLARSHIP OPPORTUNITIES ASSOCIATED WITH CAMP? Several scholarship awards are available at each camp. These include the Director, Counsellors and Peer Awards.
